This was the findings of the committee comprising chief engineers of roads and chief engineers of designs that probed into the causes of over-bridge collapse in the state capital.
The committee which submitted its report to the Chief Minister clearly stated that the SOP was not followed properly while approval for the centring for the fly-over. The report recommended major penalty for such lapses, said Works secretary N K Pradhan.
Executive engineer Dukhabandhu Behera, deputy executive engineer Bansidhar Praharaj and assistant engineer Kishore Kumar Rout were held responsible for the mishap, the report said.
Pradhan said the Panda Infra Project Pvt Ltd, the construction company which was entrusted with the work of the project, will be black-listed as it did not submit the design of the bridge before undertaking the work. Besides, the company violated safety guidelines during construction, the 132 page report stated.
